Assignment Purpose: The purpose of this assignment is to focus your learning on the role of interprofessional collaboration in case management. It is aimed at increasing your knowledge of allied professions—profession from other disciplines that collaborate with case managers in social service workers and social workers to enhance client support and service delivery. The assignment also focuses on enhancing your ability to think critically about the potential benefits and challenges of collaboration and explore best practices.
Description: For this assignment, you will work individually or in groups of no more than 3 members to prepare a research paper that explores the collaboration between social work/social service work and allied professions. Please note that if you choose to complete the assignment in a group, it is your responsibility to coordinate the collaborative process. For the assignment, you will produce a written research paper on the nature, importance and dynamics of interprofessional work in case management based on a case that will be provided.
Instructions
Step 1: Case Plan
- Review case provided below for this assignment and answer the following questions:
- What are the issues/problems faced by each family member as well as the broader family system issues?
- What individual strengths do family members possess that can be tapped into when creating an intervention plan?
- Based on what you identify as the issues, identify what services might be useful to Mrs. Brand and each of the family members. What is your rationale for suggesting these interventions? Provide at least 1 scholarly source that supports these interventions.
Step 2: Select Allied Professions
- You are the case manager working on the case provided. Select any 2 professions that you believe would be best suited to collaborate with to address the case and enhance client outcome. You can choose from the list below:

Step 3: Conduct Research on Allied Profession
- For the 2 professions you have selected in step 3, conduct research on the ethics and standard of practice, training requirement, and responsibilities of persons working in these professions in Ontario. For each profession, you will investigate and analyze:
- Professional College that governs the profession (if one exists).If no professional
College exists, you will need to note this.
- Standards of Practice and Codes Ethics governing the profession. Please note that standard of practice and codes of ethics for each profession may be exhaustive, so will need to summarize the key standard of practices and ethical principles, ensuring precision while being concise.
- Education and training requirements, including post-secondary level, required subjects, number of courses, number of hours of Field Experience. Note: If formal educational or professional registration requirements do not exist for any profession you selected, document the profession’s scope of work in Ontario and, usual training and credentials held by those working in this capacity.
- Key roles and responsibilities expected of persons working in the profession.
Step 4: Allied Profession in Case Planning
- Using the Case and the 2 allied professions that you have chosen in step 3, answer the following questions:
- Discuss what is your role and key objectives as the case manager working with the other 2 allied profession on the case. Ensure to provide specific examples from the case as well as at least 1 scholarly source to support your response.
- Explain what is the role of other 2 professions that you selected in addressing the client’s situation in the case provide? Ensure to provide specific examples from the case as well as at least 1 scholarly source to support your response.
- Based on the standard of practice and ethical guideline you have examined for the 2 professions in step 3, explain what would be the areas of the priority for 2 professions working on the case?
- Discuss the benefits of collaborating with the 2 professions on the case? Ensure to refer to the case and provide evidence from at least 2 academic sources.
- Discuss the challenges that you as the case manager may encounter with collaborating with workers in each of the 2 professions that you have selected above. Ensure to provide examples from the case and evidence from at least 2 academic sources.
- Propose at least 2 opportunities and mechanisms to address these challenges. Ensure to use Ensure provide evidence from at least 1 academic source to support your recommendations.
Submission Guidelines:
- The paper should be NO MORE than 1200 words in length (not including reference list, title page, and appendix page). Note keenly that papers that have more than 1200 words WILL NOT be graded, to encourage you to organize your thoughts concisely, improving the quality of your paper.
- Papers should be written in APA format. Images (e.g. charts) are allowed only if it complements the content and does not take more than 1/4th of a page. It is best if you place images in an appendix. Your paper should have minimum of three (3) scholarly references. - Your paper must be typed in double-spaced, use a 12-font size with 1-inch margin and completed in WORD/PDF format.
- Your paper must be submitted in the appropriate Blackboard folder by midnight (11:59pm) on the schedule deadline (see critical path for details). There will be no opportunity to make-up for missed case notes unless there are extenuating circumstances. Please speak with your instructor immediately if you are having challenges that will prevent you completing the case note by the deadline
- Note keenly that grades WILL be deducted for failure to comply with these guidelines.
- The paper is worth 25% of your final grade. Content, organization, comprehension, synthesis, critical analysis, structure, and referencing format will be some of the criteria for grading your paper. Please see below rubric for details.

The Case of Mrs. Brand and her Family
Mrs. Brand is 74 years of age and lives in a small house with her 28-year-old adult son, Trevor and 42-year-old daughter, Jade. Jade has three children, Remi, age 14, Milo, age 10 and Sarah, age 6 who also lives in the household. Mrs. Brand receives pension and old age security benefits. She has a few older friends in the neighborhood but does not otherwise have many social contacts. The local pharmacy delivers her medications to her home for her heart condition. Her son Trevor, who has Down Syndrome and Schizophrenia (therefore would be referred to as having a Dual Diagnosis), also receives medication. Mrs. Brand’s husband passed away two years ago.
Mrs. Brand takes care of her grandchildren, while their mother, Jade, works night shifts. Remi is responsible for walking his little brother and sister to school, which is about 8 blocks away from their house. Jade often picks the children up at school; however, sometimes, Jade’s brother Trevor, will pick them up and run errands with them. Trevor has on occasion left the younger children unattended at the mall, which has resulted in the shopkeepers calling the police.
About a month ago, Mrs. Brand fell down the stairs in her home fracturing her right hip, requiring surgery. Trevor found his mother lying at the bottom of the stairs hours after she had fallen. He called 911 and was found curled up beside his mother. Mrs. Brand has spent the last month in hospital having suffered a stroke. She has no use of her right side and requires 24hour support. She worries about how her children and the grandchildren will manage without her day-to-day support and financial assistance if she is placed in residential long-term care.
Mrs. Brand’s adult son, Trevor, requires extensive care of his own, as his mother is no longer able to assist him. He requires daily medication monitoring in order to manage Schizophrenia. Trevor has been known to abuse alcohol when experiencing stress and to stop taking his medication. In addition to this, Trevor has difficulty in reliably processing information, following directions and at times, making good decisions. Since his mother’s fall, the neighbors have found Trevor wandering the neighborhood late at night. Several neighbors have reported that their cars have been broken into with loose change and cigarettes missing. A new IPhone was stolen from the glove compartment of one vehicle. This theft was reported to the police. On one other occasion a neighbor confronted Trevor as he hid between two cars in her driveway after 10 pm. He threatened to punch her and break her windows if she called the police. She was unsure of what to do. It is reported that he is usually intoxicated during these times and appears confused about where to find his home. Trevor has a prior record of disturbing the peace, assaulting a police officer and is currently serving a period of probation for these offences. One of the conditions in his Probation Order is to; ‘keep the peace and be of good behavior’.
As she is no longer able to help care for them, Mrs. Brand’s two youngest grandchildren have been mostly left alone to care for themselves while their mother, Jade, is at work. The school has noticed a change in Milo’s behavior. He has exhibited aggressive behaviors such as hitting and has made threats towards both the teachers and other classmates. He has gone from a solid B student to failing several of his classes. Sarah has been found crying during school. She has recently become withdrawn from staff and peers and began pulling chunks of hair out of her head. The oldest grandson, Remi, has been skipping school, and when he does attend, he often has altercations with peers. Remi has experienced several learning challenges, and following his grandmother’s stroke, these difficulties appear to have become more exaggerated. Recently, the school made a referral for behavioral assessments on each child.
